Level 3 is a two step process:

  • You first agree to The Rules below: Sign and send.

  •  Then complete the Application below that. Sign and send.

You will receive auto-confirmation of both to the email address that you provide. Upon review, we will reach out to you with further question/instructions to proceed.

Step 1: Rules agreement

Read all of the rules. Add Name, Consent, Date and Send

1. Residents must stay drug & alcohol free. This includes use and/or possession of anabolic steroids, smart powders, kratom, etc… Drug testing will be conducted without prior notice. Refusal is considered a positive result and is grounds for immediate eviction.
2. STAY CONNECTED and it’s suggested to attend in-house 12-step meetings.
3. Must attend a minimum of four (4) outside recovery (MEN’S MEETINGS ONLY until completion of 9 month program) meetings per week. Each resident must have their meeting paper signed.   Helping Hands West- Mandatory meeting for all residents.
4. Curfew – All residents must be on the property by 10:30pm, Sunday-Friday. Saturday night by 11:30pm.
5. If you are not working or have a day off, please be up by 7:00am and attend two (2) or more groups that day. Applies to Monday thru Saturday.
6. All residents must sign out when leaving the house and sign back in upon returning. – Be specific.
7. Stealing, destruction of property & disruptive, threatening, violent or abusive behavior towards staff or residents will not be tolerated and is grounds for immediate eviction.
8. Clean up after yourself. Keep yourself and your room clean and up to staff standards. One (1) television per room under 27″ with approval from executive director prior to bringing in television.  Game consoles are allowed in rooms ONLY!   ALL Electric appliances are prohibited in rooms.  (coffee makes, refrigerators, etc.)
9. One (1) overnight visit per month after 30 days.  Submit request in writing 72 hours in advance. Staff approval is based on individual progress. Rent and house fees must be current.
10. STAY OUT OF THE KITCHEN. No cooking permitted by anyone other than kitchen staff.
11. No smoking, vaping, nicotine pouches, or chewing tobacco anywhere in the house.
12. If you have a vehicle must have valid license, proof of insurance, and current registration.
13. If you are going to miss lunch or dinner and want the meal saved for you, please sign up at the kitchen. If you do not sign up or are on the property at meal time, no meals will be saved for you.
14. You can ONLY play music in your room and only WITH headphones.  No playing of music on radios or phones in common areas of all houses.
15. Gambling of any kind is strictly prohibited on property.
16. All 3/4 residents must do Two (2) meeting watches per month. NO EXCEPTIONS.
17. Laptops and desktop computers are only permitted in your rooms & use only by you.
